Other Installment Loans			2. Recent pay stub or W-2
			3. Checking & Savings Accounts (2 most recent      statements)
			4.  If self-employed:     - If Corporation, past 2 years tax returns     - Last four 941 forms
			5.  Name & Phone number of insurance agent(s):    - Hazard Insurance    - Flood Insurance (if applicable)
