Unsecured Lines of Credit			1. Complete a San Luis Trust Bank application including a     Personal Financial Statement
Commercial Real Estate			2. Personal Tax Returns, 2 years, signed (include K-1     schedules when applicable)
			3. Business Tax Returns, 2 years, signed (include K-1     schedules when applicable)
			4. Articles of Incorporation, Partnership Agreement, Fictitious     business Name Statement, and Trust Agreement     when applicable.
			5. Checking & Savings Accounts (2 most recent statements)
			6. Other Investments or Accounts   - Copies of investment statements (IRA stmts or     brokerage stmts)
			7. If self-employed:   - If Corporation, past 2 years tax returns   - Last four 941 forms
			8. Name & Phone number of insurance agent(s):   - Hazard Insurance   - Flood Insurance (if applicable)
